She created the witch legs & shoes design with a combination of applique and fabric paint, and she shares a template and tutorial on her blog. Her no-sew method uses Mod Podge (for fabric) to attach the pieces of fabric to the shirt, but you could also use a fusible webbing applique method. Go to her post to get them.
